(2) Click the submit button on the last page of the order form and send the data through your existing email account, or
(3) Send the completed document as an attachment or scan to: ordersubmission@startouch.com
(4) Print out the completed document and send it to us via fax (360-384-3867), or in the mail to:

Note: Remember to Save a Copy for Your Records. If sending via email, you should immediately receive a confirmation email verifying we have received your order. If you do not receive the confirmation, please check your "sent" box to verify that the order was successfully sent from your computer.
